Why Do We See You Three Times a Week in Rehab?

During your short-term rehabilitation stay, your medical needs may change quickly. To keep you safe and support your recovery, a Gerio Primary Care Advanced Practice Registered Nurse (APRN) will see you three times each week.

What This Means for You:

  •  Frequent check-ins to monitor your condition
  •  Quick adjustments to medications or treatment plans
  •  Close teamwork with therapy (PT/OT/ST)
  •  Help preventing avoidable hospital trips
  •  Clear communication with you and your family

Why This Helps:

After a hospital stay, patients often have:

  •  Pain, mobility, or wound-healing needs
  •  New medications to manage
  •  Higher risk of infection or complications

Regular visits help catch problems early and keep you progressing toward going home safely.

What If You’re Not Receiving Rehab and Staying Long Term?

Each month, the Medical Director, and/or Nurse Practitioner, reviews your individual care plan and receives feedback from the Nursing Home staff. This helps make sure care plans are appropriate, concerns are addressed, and best practices are followed for each individual patient.

What Happens on Review?

During monthly rounds, the Medical Director, or Nurse Practitioner, may:

  • Review residents’ medical conditions and overall health
  • Look for any changes or concerns, such as falls, infections, or weight changes
  • Review medications and treatments
  • Work closely with nurses and other care team members
  • Help guide improvements in care and safety

The Medical Director may see residents in person or review their care in collaboration with the care team, depending on individual needs.

  • Improve communication among caregivers
  • Identify potential issues early
  • Support resident safety and quality of life
  • Ensure we meet state and federal care standards

If You Have Questions:

Please tell your nurse or therapist—they can contact your Gerio Primary Care provider anytime.

Your provider is also rounding routinely in the building – inquire from nursing staff when the provider will be in-house next to be present during a visit.
